Application

Enalapril / Hydrochlorothiazide "Orion" is used for high blood pressure where treatment with one drug is insufficient.


Dosage

Available as tablets.

Adults

  • Usually ? - 1 tablet once a day.
  • The dose may be increased to a maximum of 2 tablets a day.

Note:

  • The joint can be used to facilitate intake. It can not be guaranteed that half a tablet contains half dose.
  • Can be taken with or without food.
  • Forgotten dose :
    • The forgotten dose is skipped.
    • The next dose is taken at the usual time.
    • Do not take a double dose.

Should not be used

Enalapril / Hydrochlorothiazide "Orion" should not be used for:

  • Hypersensitivity reactions with previous use of ACE inhibitors
  • Known hypersensitivity to thiazide-type diuretics or to drugs that chemically resemble thiazides, eg sulfamethizole (antiinflammatory agent) or certain tablets against diabetes.
  • Very poorly functioning liver or kidneys.

Pregnancy

Do not use.

The risk of affecting the fetus is greatest in the 2nd and 3rd trimesters.

  • Blood pressure lowering due to both enalapril (ACE inhibitor) and hydrochlorothiazide (diuretic).
    • enalapril inhibits the enzyme that converts angiotensin I into the blood pressure enhancing hormone, angiotensin II. A decrease in the amount of this hormone causes the blood vessels to expand so blood pressure decreases. The agent also affects the salt and water secretion through the kidneys, so the heart is less affected.
    • hydrochlorothiazide increases salt secretion in the kidneys. The increased salt secretion will "pull" water out of the body. This reduces blood pressure.
  • The effect occurs after approx. 1 hour and maximum after 4-6 hours.
  • The effect lasts for approx. 24 hours.
  • The half life in the blood (T?) is approximately 30 hours.

Pharmaceutical forms

Tablets. 1 tablet contains 20 mg enalapril maleate and 12.5 mg hydrochlorothiazide.


Special warnings

The product should be used with caution when:

  • Narrowing in the body ulcer or in the pulmonary veins of the kidneys
  • Cardiac disease
  • Enlarged heart muscle
  • Connective tissue diseases
  • Gout.
  • Diabetes
  • Increased tendency to bleeding due to decrease in platelets.
  • Tendency to allergic swelling of the face, oral cavity and larynx (angioedema).
  • Potassium and sodium levels in the blood should be checked during treatment as disturbances in potassium balance may occur.
  • The dose should be reduced by poorly functioning kidneys.

Using other medicines

It is important to inform your doctor about all the medicines you are taking.
  • Cholesterol lowering agents (colestipol and colestyramine) may reduce the effect of Enalapril / Hydrochlorothiazide "Orion".
  • NSAIDs and acetylsalicylic acid in pain reliever doses (anti-rheumatic and painful agents) may also reduce the effect of Enalapril / Hydrochlorothiazide "Orion" and at the same time increase the risk of kidney damage, especially in the elderly.
  • Enalapril / Hydrochlorothiazide "Orion" may increase the effect of anti-cardiovascular and muscle relaxants.
  • Enalapril / Hydrochlorothiazide "Orion" increases the risk of poisoning with lithium (antidote).
  • Concomitant treatment with potassium, potassium-sparing diuretics (diuretics), trimethoprim / sulfamethoxazole (antibiotics) or ciclosporin (anti-rejection agent) may increase the amount of potassium in the blood.
  • In combination with Enalapril / Hydrochlorothiazide "Orion" and adrenal gland hormones, beta2stimulants (agents for asthma and COPD) and amphotericin B (fungicide) can reduce the potassium content of the blood.
  • Concomitant treatment with allopurinol (antidiabetic agent) may cause severe hypersensitivity reactions.
  • If you are taking anti-diabetes medicines, you should check your blood sugar regularly, especially during the first month of treatment.
  • If you have diabetes or impaired renal function, do not treat aliskiren (another medicine for high blood pressure).
  • Concomitant treatment with angiotensin II inhibitors or aliskiren (other antihypertensive agents) increases the risk of low blood pressure, increased potassium in the blood and kidney effects.
  • When co-administered with everolimus (cell-killers) or sirolimus (anti-transplant rejection device) there is an increased risk of allergic swelling in the face, oral cavity and larynx (angioedema).
