Explosive vomiting - what can the cause be?

Questions

Dear Network Doctor

Our 15 year old daughter has a continuous problem, she suddenly gets spontaneous vomiting, which lasts for 22-28 hours when she throws up constantly every 20 or 30 minutes sometimes the interval is shorter. It goes without saying that she gets very upset. It started 4 years ago when we were on vacation. It has often been on holiday or at other happy events. We have tried everything - avoid acidic fluids (soda), consulted your own doctor, emergency doctors etc.

She is treated with Primperan (suppositories) 10 mg. Alopam 10 mg and pamoles 500 mg. And this time we have been prescribed Imigran sumatriptan 20 mg. (Nasal spray). The doctor's words: May stomach migraine. Never heard of it. Now we are tired of various medications and no dianose can be determined.

Our daughter is by no means an anorectic girl, she is very active - cultivates a lot of sport and is very happy and outgoing. She is very fond of her school, and is a skilled student, whom we in no way stress with regard to good results, we support her and her leisure interests. We hope that someone has heard about this and can give us an answer or an idea for consideration. Looking forward to hearing from someone.

Sincerely, one asks

Dear Questions

Vomiting may be a symptom of many ailments, even outside the gastrointestinal tract, although this is relatively rare. Everyone knows about vomiting, and then often in connection with an infection. Your daughter has apparently begun to have vomiting already in the age of 10-11, and it should therefore be ruled out that there is no narrowing of the gastrointestinal tract that could be congenital. Often they will occur at the transition from one part to another part, for example. at the mouth, but it can also be seen in the duodenum. There may also be a piece of colon that does not work properly and will therefore act as a narrowing.

If vomiting occurs very often, you will lose weight, but luckily it did not happen. The only way to get this closer is to carry out a binocular examination of the esophagus, stomach ache and duodenum (gastroscopy). Depending on what you find, X-ray examinations of the remaining part of the small and large intestine (or other special examinations) may be necessary. By these methods, one tries to detect a visible change in an organ (the gastrointestinal tract), either with a binocular or an X-ray examination.

However, many disorders are functional, ie There are immediately no detectable changes, but there is a bad function. For the gastrointestinal tract, it is far more frequent than actual changes. This could be suspected in the current situation. There is a term called the "brain-intestinal axis", which you still do not know so much about besides that the brain also sends impulses to the intestine (as to so many other organs in the body). These impulses are often of an inhibitory nature, especially to the large intestine, which can lead to excessive bowel movement and can occur even if you go to the bathroom daily. This type of constipation can lead to vomiting at various intervals, and that may be the explanation. This condition can be detected by a so-called passage time study of the intestinal tract function. Thus, there must be an explanation for these vomiting, and the above plan could formally bring the cause to light. This should probably happen through a specialist or hospital department.
